DOT Physicals: What to Expect

Here’s a breakdown of what’s included in your DOT physical—and why each part matters for your health and your career.
What Is a DOT Physical?
A DOT physical exam, required by the Federal Motor Carrier Safety Administration (FMCSA), ensures that CDL drivers are physically and mentally fit to operate a commercial motor vehicle (CMV). The exam must be completed by a Certified Medical Examiner (CME) and is valid for up to 24 months.
Full Breakdown of the DOT Physical Process
✅ Blood Pressure & Heart Rate Check
Your blood pressure and heart rate are measured to assess cardiovascular health. Uncontrolled hypertension can lead to serious complications and disqualification.
Tip: Drivers with Stage 1 hypertension may still be cleared but need annual follow-ups.
✅ Vision Test
We test your visual acuity, peripheral vision, and ability to distinguish traffic signal colors during your DOT eye exam.
You must have at least 20/40 vision in each eye, with or without glasses or contacts.
✅ Hearing Test (Whisper Test)
You’ll undergo a whisper test from 5 feet away, or an audiometric test if needed. Good hearing is critical for noticing hazards and emergency signals. This is the DOT standard to maintain your CDL.
✅ Height, Weight, & BMI
We record your height and weight to calculate BMI (Body Mass Index), which helps identify obesity-related risks.
Obesity may increase the risk of conditions like hypertension, diabetes, and sleep apnea.
✅ Neck Circumference & Sleep Apnea Risk
A neck circumference greater than 17 inches (men) or 15 inches (women) is a potential indicator of obstructive sleep apnea (OSA). This condition can cause daytime fatigue and increase crash risk.
If you are at high risk, a sleep study may be required.
✅ Urine Dipstick Test
This is not a drug test. The urinalysis checks for blood, glucose, and protein in the urine—potential signs of diabetes, infection, or kidney issues.
✅ Musculoskeletal & Neurological Screening
We check joint function, strength, reflexes, and balance to ensure you're physically capable of operating a CMV safely.
✅ Medical History Review
We’ll discuss any prior surgeries, chronic conditions, medications, and mental health concerns that could affect your ability to safely perform your job.
